Rationale

uCKD and end-stage renal disease (ESRD) are significant public health problems in the United States and a major source of suffering and poor quality of life for those afflicted uThey are responsible for premature death and exact a high economic price from both the private and public sectors uCKD and ESRD are very costly to treat. Nearly 25% of the Medicare budget is used to treat people with CKD and ESRD (HealthyPeople.gov, 2018). uResources uProfessional resources include up to date education and evidence based research. Reliable resources include:   uhttps://www.uptodate.com/home uhttps://www.andeal.org/topic.cfm?menu=5303&cat=4042 u uRelation and Importance:
uMaintaining optimum health and quality of life is important for individuals of all ages.  uUnderstanding that chronic kidney disease is not only debilitating, but time consuming is important for the individual and family. u Dialysis is time consuming. Multiple hours a day for 3 days a week doesn’t have to be the course of the individuals life. This also puts a strain on family life and roles. When an individual becomes ill with disease, family roles and dynamics change.  uIndividuals diagnosed with CKD need to maintain proper and appropriate diet and avoid strenuous activities.   u uRelation and Importance (Contd.): uThe number of patients starting renal replacement therapy (RRT) is increasing.   uImprovements in the management of chronic kidney disease (CKD) across communities to alter this trend are a public health priority. With this increasing number health care cost also increases within our community and nation.  u In total, stage 2–stage 4 CKD costs Medicare FFS about $49 billion each year! See attached PDF for more staggering cost and effects on the community.

uHow to optimize health:
uEducation:
§Learning about the causes of kidney disease and the prevention is very important in working towards an optimized health. §Diabetes is the most common cause of kidney failure.  Diabetes education is essential to prevent Chronic Kidney disease  § All racial and ethnic groups have benefited equally from lifestyle modification program mentioned by the  Diabetes Prevention Program (DPP) . This program educates individuals and the community on: •Heart & Kidney healthy diets •Weight reduction •Exercise Programs u uHow to optimize health contd. uProper Diet:  §Understanding the types of food and beverages that aid good health is very important for patients diagnosed with CKD. §Dietary changes may include limiting fluids, eating a low-protein diet, limiting salt, potassium, phosphorous, and other electrolytes, and getting enough calories and vital nutrients.
§High salt intake is linked to risk factors for both heart disease and worsening kidney function. The key is to limit salt and high protein intake

uPopulation Disparities uMale §According to the National Health and Nutrition Examination Survey (NHANES), 12.1% of males have chronic kidney disease (NKF, 2016). uFemale §According to the National Health and Nutrition Examination Survey (NHANES), 15.1% of females have chronic kidney disease (NKF, 2016). uRace §As of 2013, more than 67.7% of all new end stage renal disease patients were White §26% were Black or African American (NKF, 2016). §14.7% patients identified as Hispanic or Latino (NKF, 2016). §About 0.9% were Native American (NKF, 2016). §Approximately 4.9% were Asian Americans (NKF, 2016). §Less than 0.29% did not identify with the above categories (NKF, 2016). § uCommunity and Professional Resources u Davita offers many community and professional resources for all stages of kidney disease. Davita also offers support groups for patients with kidney disease. The link below is a good source: §https://www.davita.com/kidney-disease/additional-resource uThis community website offers education on financial help for Kidney patients §https://www.niddk.nih.gov/health-information/kidney-disease/kidney-failure/financial-help-treatment uEducation for the patient and family is vital for them during this life changing process. This website offers education for both §http://www.dpcedcenter.org/patient-family-resources u uHealth Related Organizations uThe National Kidney Foundation
